My Care Bridge, an online portal for new referrals to children and young people’s autism and attention deficit hyperactivity disorder (ADHD) services in mid and south Essex, is going live on the 28 August 2024.

Educational settings (including nurseries, schools, and colleges) will be able to make new referrals, track referrals, see what parent and education information is outstanding, add other supporting documents and videos, download the form to share with parents before submitting, as well as a ‘save later’ option, if you need to find out more information.

GP’s, health visitors and other allied health professionals will also be able to refer using the portal, without waiting for a school report, as the portal will automatically send for information from the school that professionals have named.

This new portal aims to streamline and speed up the referral process, making it more accessible to educational settings and families.

Parents and carers will be able to access the portal to provide information for their child’s referral, track the referral, and access a library of support available locally.

The portal is available for referrals for children and young people who are aged up to 18 years old, and who are registered with a GP in mid and south Essex. This includes Chelmsford, Braintree, Maldon, Castle Point, Rochford, Southend, Basildon, Brentwood and Thurrock.
